Integration of Care

An improved patient journey that is fully integrated across all services is a key goal of the WCMICS work program.

Key Work Areas:

  • Strengthening Multidisciplinary Meetings (MDMs) Program

      Multidisciplinary care, with regular prospective multidisciplinary team meetings at its core, is a key enabler of high quality, consistent and coordinated cancer care.  Studies have shown that cancer patients who are managed by a multidisciplinary model of care are more likely to have evidence-based treatment and therefore better survival outcomes, have increased recognition of their supportive care needs, and better satisfaction with their care.

      Multidisciplinary care is a central theme of the Government’s cancer reform agenda.  The long-term aim of the strengthening MDMs Program is that all WCMICS cancer services will conduct regular high quality MDMs providing the opportunity to plan treatment and care for all cancer patients.

  •  Care Coordination

      Care Coordination for patients and their families is central to the Government’s cancer reform agenda and the topic of the Victorian Government’s policy document Linking Cancer Care – A guide for implementing coordinated cancer care.  It is also a key enabler of high quality, comprehensive, consistent and coordinated cancer care, information sharing, and relationship building within and between primary, secondary and palliative care service providers.

      The WCMICS has a dedicated work program aimed at improving care coordination across the region, including the Care Pathways Projects and the establishment of the Cancer Services Positions.
